Direct Support Provider – Basic Duties
Personal Care Support:
- Assist individuals with daily living activities (bathing, grooming, dressing, toileting, eating).
- Encourage independence while providing necessary assistance.
Health & Safety:
- Monitor for medical needs, administer medications (if trained/authorized), and report health concerns.
- Ensure a safe, clean, and supportive living environment.
Skill Building & Independence:
- Support individuals in learning life skills (cooking, cleaning, money management, communication).
- Promote self-advocacy and personal growth.
Community Integration:
- Accompany individuals to appointments, work, social, and recreational activities.
- Encourage participation in community life to build inclusion and independence.
Behavioral Support:
- Follow individual support plans (ISPs) and behavior support plans.
- Provide positive reinforcement and de-escalation when needed.
Transportation:
- Drive or assist with public transportation to appointments, employment, and community events.
Documentation & Reporting:
- Record daily activities, progress, incidents, and health information accurately.
- Communicate observations and concerns to supervisors, care managers, or families.
Advocacy & Rights Protection:
- Ensure individuals’ rights are respected.
- Support choices and self-determination.
